Toyota's VVT-i system uses oil pressure to advance the intake cam via a sprocket-mounted actuator. The OCV (oil control valve) sits in the cylinder head and contains a fine mesh filter. When that filter blocks with oil sludge — common on cars run on 15,000-mile intervals — oil flow to the actuator falls and the ECU logs P0011 because commanded advance can't be achieved. The 2AZ-FE is particularly prone due to head gasket weeps mixing coolant with oil.
