Upstream (pre-cat) oxygen sensor on bank 1 is stuck reporting a lean mixture — the ECU knows the sensor signal is frozen because the fuel trims are all over the place trying to correct a fault that isn't really there. It's a very different code to P0171 (genuinely lean) even though they get confused constantly. On UK cars it's usually a tired lambda sensor past 90,000 miles, an intake vacuum leak the ECU is chasing, or a wiring/heater-circuit fault that stops the sensor switching properly.

The P2195 misdiagnosis that wastes money
P2195 is one of the codes garages love to throw a £180 lambda sensor at without doing the basic test first. Bob's 32 years on the tools rule: NEVER fit a lambda sensor for P2195 until you have three answers. One — is there a vacuum leak? On Vauxhall A14NET the plastic intake manifold cracks around the injector bosses and leaks air; on VAG EA888 the PCV diaphragm in the rocker cover splits and pulls a huge vacuum leak that no smoke-test will show unless you deliberately pinch the breather. Fix the leak first and P2195 will clear on its own. Two — is the heater circuit working? Read live-data 'O2 sensor heater duty cycle' on a scan tool with lambda coverage — if it's stuck at 0% or 100%, the sensor's heater has failed and the sensor never reaches its 350°C+ operating temperature. That's a genuine sensor replacement, but it presents as 'stuck signal' rather than 'lean mixture'. Three — is the sensor itself carbon-fouled? On any direct-injection engine (Mini Prince, Ford EcoBoost, VAG TSI, Peugeot PureTech, Kia GDi) the pre-cat lambda gets coated in carbon from intake valve deposits — a JLM or Wynn's intake cleaner + a proper Italian tune-up (30 minutes of 3,500+ RPM under load on a fast A-road) sometimes recovers a sluggish sensor for another 20k miles. Only if all three tests point to the sensor itself do you fit a replacement — and always OE-spec (NTK, Bosch, Denso to the original part number) never a £22 eBay universal.

The tell-tale that separates a genuine tired lambda from a wild-goose chase is the fuel-trim graph. Ask the garage (or read it yourself with Torque and a decent OBD dongle) for 'short-term fuel trim bank 1' and 'long-term fuel trim bank 1' at idle, warm. On a healthy engine short-term flicks between about -5% and +5%, long-term sits within ±7%. If long-term is stuck at +20% or higher and short-term is bouncing off +25%, you've got a real lean fault — a vacuum leak, a fuel-pressure problem, or a genuinely lazy sensor. If long-term is close to zero but the code still stores intermittently on a motorway run, the fault is almost always the sensor heater or the wiring — not the mixture. On Mini Prince and Peugeot PureTech engines, a walnut-blast of the intake valves (£220–£380) transforms P2195 cars and also fixes the misfire-under-load complaints that usually come with them. On VAG EA888 always change the PCV valve as part of the rocker-cover job — it's a £30 part on a £400 job, and the whole PCV/rocker-cover assembly is the failure point.
